Your First Visit
Welcome to Altitude Oral Maxillofacial Implant Center! Our doctor and team members are looking forward to helping you have a positive, fulfilling experience as our patient. After we’ve confirmed that you’ve completed the necessary new patient registration forms (either here in-practice or online), you’ll be escorted back to the treatment room so that we can capture X-rays, conduct your consultation, and perform surgical care as needed. Dr. Fangman enjoys chatting with patients and learning more about their lives, their reasons for pursuing oral surgery, who referred them to the practice (if they did receive a referral), and more. Our hope is to form a solid, respectable relationship that will carry on throughout the consultation and treatment process!
Patient Forms
Instead of having to wait until you’re actually in our office to complete our paperwork, we’re happy to make this process much simpler for patients by offering online registration! This can be completed at any point before your first appointment and in the comfort of your own home. Plus, once you actually arrive for your consultation, you’ll be able to see the doctor that much faster!
Insurance Accepted
Pursuing necessary oral surgical care shouldn’t have to be an overcomplicated or financially stressful process. That’s why our team at Altitude Oral Maxillofacial Implant Center accepts multiple methods of patients, including many major insurance plans – both dental and medical! We will assist you with all of the necessary paperwork so that claims are filed quickly and successfully. Acceptable plans include Ameritas, Cigna, Delta, MetLife, Aetna, and more. You are always welcome to contact us to learn whether or not we take your specific plan.
Financing Available
If you don’t currently have viable dental/medical insurance or if you need to break down the overall cost of your treatment into multiple payments, affordable financing can be arranged through CareCredit! These plans come with little to NO interest, depending on the length you select, and they can help you fit the cost of surgical care into your monthly budget without difficulty.
